Medical Oncology Medications

Health Advantage utilizes Carelon Medical Benefits Management, an independent company, to provide prior authorization of health plan coverage for medical oncology medications.

Please review the member’s benefit certificate or call the customer service number on the back of the card for a listing of Health Advantage plans who utilize the Carelon medical oncology program.

Providers are required to secure a prior authorization for medical oncology medications. Providers may submit a request to Carelon Medical Benefits Management.
