Accordingly, we offer a number of resources that are intended to support our valued healthcare providers in addressing the needs their patients may have in the areas of mental and behavioral health, including substance use.

Behavioral Health Team
Our Behavioral Health Team at Health Advantage is made up of dedicated professionals, including nurses, social workers and peer support specialists, who work alongside providers to ensure your patients receive comprehensive care that addresses both their behavioral health and overall well-being.

Our nurses are trained to manage both behavioral health and primary medical conditions, providing an integrated approach to care. They offer personalized support for patients with comorbid conditions, ensuring that their physical and mental health needs are met for a more complete healthcare experience.

- Do you have patients whose struggles with addiction or opioids require more help than you can give? Good news! You now have a convenient and effective virtual tool to help your patients who are Health Advantage members deal with substance use issues.
- Referring to Bright Heart Health – If your patient is ready for assistance with their substance use disorder you can refer them for this free covered health benefit by any of the following methods:
- Bright Heart Virtual Clinic – brighthearthealth.com/contact-us. Staffed 24 hours a day, 365 days a year and uses HIPAA-compliant Zoom videoconferencing.
- Phone: 800-892-2695. Speak to an intake staffer or leave a message and receive a call back.
- Online referral form – brighthearthealth.com/intake-forms/patient-referral/. Write as much as you’d like about the patient (diagnosis, medications, etc.).
- Fax: 415-458-2691 – Send pertinent medical records, along with a cover sheet providing the patient’s name, birthdate, phone number and email (if they have one).
